Once you have submitted your membership application, you must submit the following to hr@laurelbrook.org:

 

  1. Background Check Authorization Form (click here to download)
  2. Copy of Passport/Government ID
  3. Educational credentials (diploma, certificate)
  4. Resumé
  5. Pet Policy Form (if applicable) (click here to download)
